Transcripts are protected by FERPA, and are only released with the written consent of the student/graduate or otherwise permitted under FERPA. Case Western Reserve University considers the grades earned by students and other information about performance at the University to be a private matter. Please complete and submit a transcript request form either in person, by mail or by fax to have a transcript of grades released. Transcripts will not be issued to, or on behalf of, students who have not discharged all delinquent obligations to the University. A $5 processing fee is charged for each transcript copy; $10 for each faxed copy. Faxed copies are not considered official documents. Please verify with the recipient that a faxed copy is acceptable.
It is the policy of Case Western Reserve University to include all academic information from all levels - undergraduate, graduate, and professional - on the official transcript.
If you are a School of Medicine alumni applying for a fellowship through ERAS, the Office of the Registrar can upload your transcript and Medical Student Performance Evaluation (MSPE, formerly known as the Dean's Letter) directly to the ERAS Fellowships Documents Office (EFDO) through the Medical Institution Document Upload Service (MIDUS). In order for our office to upload a document using MIDUS, you must send an electronic request using EFDO On-line Services.

Once you submit your request in the EFDO On-line Services, our office will receive an email with a link to access MIDUS and upload instructions.
You will also need to submit a Transcript & MSPE Request Form, either by mail, fax, email, or in person, before your transcript and MSPE can be uploaded through MIDUS.
Current students can generate and view their unofficial transcripts by following these instructions.
The Office of the Registrar provides documents to support verification requests for licensure, M.D. degrees and enrollment for graduates of the Case Western Reserve University School of Medicine. Please submit a request in writing by fax to (216) 368-4621 or by mail. Do not submit a request more than once. All third party requests for verification must include a consent to release information.
Please be advised that the SOM Registrar's Office can only verify graduation from the M.D. program. We cannot verify post-graduate training, such as residencies and fellowship, or practitioner staff appointments. Any requests to verify post-graduate training or staff appointments should be directed the appropriate office listed below:

For faster processing, third parties are encouraged to identify the specific department and contact information and send the request directly to the individual department with a signed release to expedite verification. Our office does not carry directory contact information for individual departments; please refer to the websites listed above for departmental contact information.
To obtain verificatin of a degree, other than an M.D. degree, earned at CRWU, please contact the Office of the University Registrar. Information about requesting a degree verification is available on their website at http://case.edu/registrar/forms/verification.
