registered through the SIS (Student Information System via the Registrars Office) to enter this elective in the CAS system so faculty can complete the EOR (End of Rotation) form for final Grade Submission.

review materials listed in the references section for the course; these
materials can be made available upon request or through the Health
Sciences Library.

questions before moving on to the next section.

- OPTION CHANGE OR DROPPING ELECTIVE:
APPROVAL: Dr. Robert Haynie or Dr. Kent Smith's signature is required for Registrar's "Add/Drop Form",
whenever dropping or making any revisions to your original registration.
NOTIFY: Dr. Maryrose Baushka and Patti Quallich of any revisions to your original registration for the course options or if you have dropped this elective, so they are aware of your course requirements.

SURVEYS / EXAMS |
|
- PRE-COURSE & POST COURSE SURVEYS:
To evaluate this course,
we need your comments and suggestions. The Pre
and Post-Course Surveys are linked to the online exam. Your answers to the survey will not affect
your grade for the course. On the Module/Exam Introduction Page press the lower right button "START EXAM" to begin your answers to the Pre-Course Survey. After taking the Pre-Course Survey and exam, you will be required to fillout the Post-Course Survey before you are able to submit your exam.
The exam system will work on both MAC and PC computers.
EXAM REQUIREMENT:
All students must complete the eEXAM upon completion of the course. This exam can be taken at any point during the elective after the online material has been read. You may access the exam as many times as you want since there is no time limit. REMINDER: Save your answers often.
The exam system will not allow your final answers to be submitted until the Post-Course Survey is completed.
Choose the "SUBMIT" button ONLY when you are ready for evaluation.
NOTE: After the academic year ends (June 30th), there will be no access to the online exam/module system or after you submit your exam for evaluation.

| FOR 2 WEEK OPTION: |
Requires studying the online CHAPTER MODULES and taking the pre-course and post-course surveys and the online exam to complete the elective.
Those taking the two week option are not required to complete a written module.

CHAPTER MODULES: |

Requires reading and studying these online modules, taking the pre-course and post-course surveys, taking the online exam, and writing a module to complete the elective. The exam can be accessed even before your written module is completed and submitted online.

CREATING YOUR MODULE: |
|
MODULES:
Students
taking this option are required to contribute to the development of the curriculum through the creation of an educational module on prevention/health promotion in an area of interest. Follow the existing modules as a template for the format. Students contributing to the
course curriculum will be considered for honors credit.
NOTE: As of July 1, 2008 the written module became a requirement for the one month option, however they can qualify for honors after evaluation of their project.
TOPIC SUGGESTIONS:
MODULE SUBMISSION:
Enter and work on your written module through the online exam system by choosing the " START MODULE" button is at the bottom of the exam web page. Your module will become part of your STUDENT PORTFOLIO. Choose the " SAVE" button to save your work so you can return to do more work.
NOTE: Enter module text directly into the online system rather than copying from Microsoft Word. When text is copied directly from the word program and pasted directly into the online system, this will place a lot of html tags not recognized by the html editor. Unless you understand html editing this method is not advised.
Images & Table Images need to be in the .jpg format, inserted separately from the text. Hyperlink your images to the web site from where you obtained them.
Choose the "SUBMIT" button when your module is completed and ready for evaluation. Modules will be evaluated by the elective director.

- CONSENT AND WAIVER FORM: Students who receive "Honors" and choose to contribute to the curriculum must sign a CONSENT AND WAIVER in order for their
materials to be added to the online course.
