Secondary Prevention (continued)


  • Surgery
    • Surgery is reserved for patients with BMI >/= 40 or BMI >/=35 with significant co-morbidities who have failed previous weight loss methods
    • Both banded gastroplasty and gastric bypass have proven to be effective in producing significant sustained weight loss for at least 5 years.10
    • There are several complications associated with surgery including:
      • intraoperative complications (10%): anastomotic leaks, subphrenic abscesses, splenic injury, pulmonary embolism, wound infections, and stoma stenosis
      • postoperative complications: incisional hernias, gallstones, dumping syndrome, weight loss failure, B12 and iron deficiencies
      • Mortality (<1%-4%)
    • In 2002 there were roughly 80,000 bariatric surgeries performed in the US with an average cost of $15,000-$30,000.
