Anonymous Animal Incident
13. Policy on Reporting Concerns Relating to Animal Use (Animal
Incident).
Concerns about animal misuse may be reported to the IACUC by telephone,
e-mail, or by completing an Animal
Incident Report (Acrobat .pdf file). These forms are available
in the animal facilities, in the ARC office and in the IACUC office.
Completed forms can be signed or submitted anonymously to the IACUC
Coordinator (School of Medicine, room WG-78). The Coordinator will
forward any concerns to the Committee for review. The IACUC
is required by regulation to protect the identity of reporting individuals
who wish to remain anonymous. The procedure for processing animal
