INSTITUTIONAL ANIMAL CARE AND USE COMMITTEE

FOR NEW INVESTIGATORS

In order to work with animals at Case Western Reserve University, you will first need to submit an IACUC Protocol Form to the IACUC office. A completed Occupational Health Questionnaire is required for all personnel listed on the protocol. If you are using chemical or biological hazardous material, the appropriate Chemical or Biological Hazard Use form must be submitted and approved

FOR CONTINUING INVESTIGATORS
WITH AN APPROVED IACUC PROTOCOL

All new personnel who participate in a protocol, and who wre not listed in the original protocl, must be added to the protocol by using the  Addendum to Add Personnel to a Protocol.

If you are using a biohazardous material, the appropriate safety form must be submitted and approved:

• Carcinogen form
• Radioisotope Form
• Pathogen Form
• Recombinant DNA form
