FACILITY ACCESS |
To obtain access to animal facilities at Case Western Reserve University, each researcher must be listed on an approved IACUC protocol and must have completed training appropriate for the protocol and for the type of animal facility they will be working in:
- A researcher may be added to an existing protocol by completing the Addendum to Add Personnel to an Approved Protocol for all protocols submitted prior to November 2009. All hard copy amendments should be submitted to Theresa Bridges in the IACUC office, specifically room WG-77.
- Protocols submitted after November 2009, log into iRIS (https://cortex.case.edu) and complete the form located within your protocol IACUC Addenda to Add Personnel to an Approved Protocol. If you have questions regarding the amendment process, please contact Theresa Bridges in the IACUC office at tdb3@case.edu or 368-6979.
- Obtain a Case ID card if you do not already have one. If you are an employee of one of the affiliated hospitals, your department must request a Case ID for you from Access Services. For access to Animal Facilities in the University Hospital Buildings your will need to obtain a UH ID Badge. Complete this form to request a UH ID Badge. Please take the completed University Hospitals Badge memo to the Medical Center Service Building (MCCO), Human Resources Department on the 6th floor.
- To activate your required participation in the Occupational Health Program, you will need to complete the Case University Health Service Occupational Health form
- and fax the form directly to the University Health Service office at fax: (216) 368-8530. If you are a CASE employee, it is possible that you may have already completed this form during your orientation class.
- Complete the Training Requirements for each facility for which you requested access. See the lists below. Access the online training modules. You will need your CASE ID and password to access the online training.
- Schedule your Facility Specific on site Tour(s) and Training.
- Contact the Training and Compliance Manager, Tami McCourt or call her at 216-368-4972 for protocol specific training and scheduling information.
