Anonymous Reporting of Animal Welfare Concerns
Any concerns about animal misuse may be reported by completing an Animal Incident Report Form. These forms are also available in the animal facility and in the ARC office. Completed forms can be signed, or submitted anonymously to the IACUC Coordinator (School of Medicine, room WG-77 or WG-78). The Coordinator will forward any concerns to the Committee for review. The IACUC is mandated to protect the identity of reporting individuals who wish to remain anonymous. The form and procedure provides
for anonymity through the use of a pseudonym. Reporting individuals who wish to inquire about their reports should use the pseudonym in correspondence.
Procedure for Handling Animal Incident Reports
- The completed form should be submitted to the IACUC Office, School of Medicine, room WG-77 or WG-78.
- The Coordinator forwards a copy of the report to the attending veterinarian and to the Chair of the IACUC.
- The attending veterinarian conducts a preliminary investigation and attempts to resolve any problems found.
- The IACUC subcommittee on compliance reviews the findings of the attending veterinarian if the issue cannot be resolved and determines appropriate action. If the incident involves an issue of noncompliance, this is brought to the full IACUC.
- If needed, the IACUC deliberates and takes action. Every effort is made to resolve the issue within sixty days. The relevant regulatory bodies and funding agencies are notified if necessary.
- The person who submitted the form may request information on the case from the IACUC Coordinator.
- A copy of the incident report and its resolution is maintained in the IACUC Office.
