Wolstein Animal Facility

 
 

Faciltiy Access

 

All investigative personnel who are granted access to animal facilities must be listed on an approved protocol. Every individual working with animals must be enrolled in the occupational health program and must have completed a Facilities Access Heatlth Training (FAHT) form. If you are already on an approved protocol, you will most likely have one of these forms on hand. Each animal housing room in the Wolstein Animal Facility (WAF) has a separate key. Investigators in a particular room will have to submit a key request. Follow the links in the description below to the appropriate forms. In order to be granted access to the Wolstein animal facility investigators will have to follow the procedures as follows:

 

A. If you

    1. Have a Facilites Access Health and Training (FAHT)  form on file

    2. Presently have access to another animal facility and

    3. Have previously attended a micro-isolator training class

B. Then

    a. Complete the WAF Access Memo  and submit it to the IACUC office


    b. Complete the WAF on-line orientation training module


    c. Schedule a tour of the WAF with Lonnie Thomas, WAF Manager

 

    d. Submit a Key Request for your animal room identified by Mr. Thomas

 

C. If you do not have a FAHT form on file:

    a. Complete the FAHT form as indicated


    b. Complete a WAF on-line orientation training module


    c. Complete Occupational Health and Training on-line training module


    d. Arrange for and attend a micro-isolator training class and facility orientation tour

        with Cheryl Urban
