Virtual Purchase Requistion


The Virtual Animal Purchase Requisition is provided below. Rolling over the fields on the form causes a brief explanation of each field to appear. Requistions are filed by this number.  Always refer to this number when making inquiries. Retain the blue copy of the requisition until the animals have arrived. The Purchase Order Number is provided by the ARC.  Leave this blank Name of Principal Investigator.  Include a contact person if applicable.  i.e. Scientist, JQ/Johnny Postdoc Provide department name An accurate phone number  is essential in case there are problems with an order. Animals can not be ordered without an active protocol with animals remaining on the the protocol. Used by the ARC to document date the order is placed Used by ARC to identify vendor for payment This documents the individual we talk to at the vendor in case there are any problems ARC Handling charge added to all orders Don't forget to sign the form Vendor adds this charge in some cases The cost of shipment is billed by the vendor and added to the invoice Total amount charged to investigator Completed by ARC personnel to confirm arrival and billing information Date the requistion is submitted.  Don't forget to time and date stamp the requistion in the main office. Specify the day you want the animals to arrive. This is for investigator records when funds need to be traced back to a named account An animal housing room must be included on every requisition before it can be processed Account number must be active and have funds in the account Always provide your vendor preference if you have one Let us know if you have any special requests to care for the animals when they arrive.  Provide an e-mail address Specifiy how you want mice to be housed when they arrive Indicate the Number of animals required Mark Male, Female or Either as applicable Only required if a specific size animal is important The age of the animal should be given as a range i.e. 6-8 weeks Use the exact nomenclature in vendor literature.   Jax animals must be ordered using a catalog number Completed by the ARC Leave Blank
