Who can request animal transports?
Any member of the investigative staff needing animals transported from one facility to another.
What form do I use?
Use the pre-printed form labeled "Animal Transport Request". These forms are available on this web site (Transport Request) or, in the ARC main office room TB03A, in the form que outside the WRB front office SB-124 or outside the Manager's office in the Health Sciences Animal Facility, room EB06B.
What if I am not sure how to complete the form?
You can ask one of the Husbandry Managers or the Training Coordinator.
What if my animals are not delivered when I requested?
Contact Keith Fields at 368.2314. He will look into the situation and call you back as to the status of your request. Generally requests are completed within 48 hours, but if the department experiences equipment failure, there may be a slight delay.
When should I fill out the form ?
As soon as you know you need animals transported. This allows adequate time to schedule and deliver your animals. The ARC requests 48 hours notice for standard deliveries.
When will my transport be fulfilled?
ARC requests 48 hour notice for transportation requests to prepare and deliver the order. If you can give more notice, please do so.
Where do completed forms go?
Please fax the form to the manager's office (216-368-5381) or they may be placed in the managers' office mailbox on their door (TB06B). Our driver is trained to check the fax machine a few times a day for orders. Placing the order in the wrong place can result in a delay in filling your order.
Where will my animals be picked up from?
In most cases your order will be picked up at your facilities designated staging area. In the ARC the staging area is located in EB03K. Wolstein has a staging rack located outside of the managers' office (S-122). You should place your animals into the staging rack the evening before the transport.
Where will I find my requested transport?
In most cases your order will be located at your facilities designated staging area. In the ARC the staging area is located in EB01K. Wolstein has a staging rack located outside of the managers' office (S-122).
Is there a fax machine inside the Wolstein Animal Facility? Yes, there is a fax machine located in room SB159 of the facility, fax # 368.3408. In order to fax your request you must dial 9.368.5381 and wait for your conformation sheet to be printed. Once your transmission is complete you can leave the original request in the Docu-Pocket that is attached on the outside of the door.
